Complete the 3 pages and bring them with you to your massage appointment. Copies of the paperwork will be provided if your not able to print them off and complete before arriving at your session. The information on the intake forms is designed to inform the therapist so you will receive the best massage. It is very important that you provide accurate information about your past and current health conditions. We do understand that often times paperwork is not required at all massage establishments. Five Star Massage is a proactive medical massage practice. It is important that the forms are taking seriously and completed prior to treatments. The two intake forms are required for anyone to receive a therapy session at Five Star Massage.
